Position: MTM Pharmacist
Position Type: Full Time
Locations: San Diego & Orange County, CA
Description:

• Contact patients and review their medication profiles.

• Identify gaps in care, high risk medications, drug interactions, and duplications in therapies. 

• Prepare case reports for patients and their primary care providers outlining clinical recommendations.

Requirements:

• Pharm D degree from an accredited college.

• Active pharmacist license in good standing.

Preferred Skills:

• Previous MTM or Medication Regimen Review (MRR) experience.

• Ability to speak multiple languages.
